Important changes & notes

No photo upload: your photograph and fingerprints are captured at the Passport Seva Kendra (PSK/POPSK) counter. The instruction booklet says a photograph is not required for applications submitted at a PSK.
Uploading supporting documents online is optional. After submitting the form you can upload them yourself or share them from DigiLocker; otherwise the counter staff scans them at the PSK.
The portal does not publish a size limit for uploads. Make clear, readable PDF or JPEG scans; if the portal rejects a file as too large, use Compress PDF or Compress image.

Take a clear photo in good light, or scan the paper (signature, thumb impression, declaration) at 200 DPI if you have a scanner.
Upload it in the matching box below. Rotate and adjust the crop so only the face, signature or impression is inside.
